At Parkside Primary Care, we believe that quality healthcare should be accessible, affordable, and personal. That's why we're excited to introduce our Direct Primary Care (DPC) model, which empowers you to take charge of your health and well-being.
With our DPC model, for a monthly fee of just $65 (additional family members can be added at a reduced rate), you gain access to a comprehensive range of primary care services. This means you can enjoy unlimited visits and consultations with our dedicated Family Nurse Practitioner, ensuring that your healthcare needs are met without many of the constraints of traditional insurance models.
Here's what you can expect from our DPC model:
✔ Unlimited office visits and consultations with our skilled Family Nurse Practitioner.
✔ Personalized, patient-centered care that focuses on your unique health goals and needs.
✔ In person and tele-health visits available to address your health concerns promptly.
✔ Extended appointment times, allowing for in-depth discussions and thorough assessments.
✔ Coordination of care, including referrals to specialists and assistance with managing chronic conditions.

Family Nurse Practitioner
Collin is a doctorate-prepared Family Nurse Practitioner with diverse clinical experiences including inpatient critical care, outpatient clinics, telehealth, and urgent care. His commitment to cultural sensitivity, advocacy for underserved communities, and dedication to education shines through his approach, ensuring every patient receives compassionate, inclusive, and informed care. He is also proud to serve as a professor at the University of Cincinnati, teaching the next generation of healthcare providers.

Many practices have thousands of patients on the roster and those providers can only take a few minutes for each visit. Parkside Primary Care is committed to a small capped number of patients so you will never be “just a number”. Also, because of our scheduling policies, you should never have to wait long to be seen for your appointments.
